The article on the Hobo handbook is interesting, but I think the writer missed one point.

Denture-cleaning tablets are often good for cleaning enclosed spaces. Fill with water, drop in several tablets, wait a few hours, then rinse.

I've found that filling it with a half and half mix of water and vinegar, and leaving it overnight, works quite well. Then rinse it out thoroughly with warm water and see if you can "hang" it upside down to dry out completely before putting the cap back on. If that doesn't do it, repeat, and maybe add a dash of bleach to the mix?

I've never cleaned a hot water bottle, but I do leave them open to dry between uses and for a very long time (many days) before storing them.
