While it didn't explode, per se, that I know of, my husband had a Pyrex item shatter in his hands when he was younger. It doesn't surprise me that someone's finding that these glass cookware items are prone to doing so.

Putting hot Pyrex on a cold or damp surface makes sense to me as a cause for shattering (pretty simple, there - cold surface cools the glass unevenly, the brittle nature of glass doesn't allow for that kind of uneven cooling, and the glass breaks), and I've never cooked anything other than baked goods (namely, brownies) in them, since I have stoneware for cooking and the glass is largely for things like measuring, but it never would have occurred to me, either, to put water in the dish when cooking meats or vegetables.
