Really, I think it's a horrible way to die. However, if, as the comments indicate, this is very common, it raises an important question.

Why haven't we duckling-proofed our sewer grates yet?

Are those gaps even big enough for them to slip down through? Looks like it'd be a tight squeeze, not just a PLUNK, baby in the sewer type thing.
