Pick a daily bedtime and a rise time. Don't go to bed before bedtime. Don't go to bed until you're tired enough to fall asleep. If you don't fall asleep withing 20 minutes, get out of bed. Always get up at your rise time. No matter what.

Well, unless you think there's a medical explanation, they're probably going to refer you to a sleep psychologist, who will tell you similar to what I told you. That's who I got it from.

It sounds like a sleep phase disorder (the blind can have a particularly nasty version though I think some sighted folk can have it, too). We basically travel around the clock as far as sleep/rise times go. I've had some version of it from shift work and being a night owl forced into a day schedule since my teens (I was a night owl even as a child).

Oh (edit) forgot to mention it's the same for me as far as being especially tired in the days before and during that time go. Exactly the same. I think hormones can play in as well, or at the least worsen whatever sleep irregularity might already be in play that might otherwise be more easily tamed.
