I've found that filling it with a half and half mix of water and vinegar, and leaving it overnight, works quite well. Then rinse it out thoroughly with warm water and see if you can "hang" it upside down to dry out completely before putting the cap back on. If that doesn't do it, repeat, and maybe add a dash of bleach to the mix?
