Never. (Basic climbing safety is something I figured out young: make sure you have three secure points before moving the fourth.)
Of course, not every kid is going to figure this out independently, but I don't remember anyone being OMGKILLED falling off the monkey bars at school either. (A couple wind-knocked-out-of-them, is all--not even worth a visit to the nurses' office.)
